That depends on how much energy she expends. Depending on WHAT she eats, if anything, you can count using magic or power as expending energy.
If you look at runners, swimmers, and people who use up huge amounts of energy, they are bone thin. (With general amounts of muscle depending on what they eat). I run 15-30 miles a week. I can pretty much eat anything I want and stay really thin.
That said, Flandre could be bone thin from lack of subsistence, or from the amount of energy she burns. If you go by this, there is good reason there are no "fat" people in Gensokyo...

Youkai a spiritual beings anatomically and so even if they do need to eat the digestion process can be different. Humans physiologically burn calories for muscles existing then burn more to make said muscles move. Youkai anatomy and physiology in Touhou is not stated and so I will not assume that it is the same.

As I said once before "Flandre has no idea how to feed directly." But Remilia may have drank youkai blood based off her move set in Scarlet Weather Rhapsody "Vampire Kiss." So it can be feeding off the essence that is associated with blood.
has anyone really just took a thought that maybe Flan would just be Skinny... A Child-body Flan is just skinny. Thats it
That's fine for regular children, but the way Yohane draws Flan makes her look undoublty skinny as if she wasn't being fed properly. Which brings up the discussion in the first place.
