From left to right: Haruka looses his memories and personality and gains the "konoha" persona body, showed in the screen here, which is superhumanly strong and fast.
Takane Looses her body and turns into a cybernetic being called ene, what we see in the sceen here is how she looks.
Shintaro comits suicide by stabbing his neck. In route XX he suicides because he never got past the suicide of his best friend, in route 01 he kills himself, after remembering everything, in order to open a Kagerou Haze and look for his long lost friend.
Ayano "killed" herself in order to save her siblings. She knew that if she died at that particular moment a Kagerou Haze would open, save her and give her an ability, since the big bad ploy depended on killing all the ability users in one place at the same time, she trapped herself inside the Haze so it would be impossible to reunite all the ability users, thus saving his sibling (who are ability users). No data means that her whereabouts are currently unknown as she's lost somewhere inside the Haze

Actually no she is in the real world with all of here friends in the anime at least and what route xx and rout 01 never heard of that.I question way didn't Azami and the others trapped in the other world didn't came back as well i fiend no reason not too.
