When I look at the picture, I see a dude holding the severed head of the person in the painting.
Seeing how the series's name is Fallen Kings and the picture's title is Fallen Knight,I think that's pretty likely. Also,when I saw Steak's comment,I thought the girl's head is still alive and being totally cool about it.
I assumed it was a case of someone helping a dullahan out. The series tag doesn't seem to have a lot of character death or morbidity. And a "Fallen King" becoming a dullahan is appropriate.
Why would you even carry a head around? And look at how carefully the head is being carried in the first place? It's like the knight is trying to keep her comfortable.
Maybe the commentary for the picture is way different from my interpretation, but I don't think in a vacuum it's far off.

The knight may be one of the vassals. The sword has no blood on it so it can be taken that he or she has returned a day late only to see a severed head and look upon the portrait. One day is enough for the blood to congeal and cleaning a corpse for burial isn't unheard of.
