The more direct translation: "You look just like a girl!!"
Though if you are saying that to a girl it might as well be the same thing.

Considering the pair, she might not find it an insult at all and some sort of a weird japanese thing.

Onna mitee (elongated vowel) literally means that, yes, but is generally used in the context of dissing another male. (like say, someone who likes to cook, or dresses inappropriately, or just plain trash talk)

So, sissy would be the most appropriate translation.

Plus in more recent usages it also has some trap undertones, which also fits some... 'fringe' uses of the word "sissy".

An alternative translation would be "effeminate", but that doesn't quite carry the same oomph, and isn't quite as 'rough' or colloquial as the original.

Regarding that, even "looking like a girl" would sounds just as much of an insult to booth sexes, to me at least.

But man, she looks more like a boy here. Guess girly stuff doesn't suit her that much.
