Well, from my understanding, an EX Rank isn't necessarily greater then A Rank. It's a Rank that falls outside the numerical system, or to put it plainly, it cannot be accurately gagued with in the normal way. So Kyohime's Mad Enhancement isn't a deeper insanity, rather it's so twisted that the usual parameters doesn't apply.

Well, to the extent you can gauge insanity to start with I guess.
