I thought the "When you see it..." comment was in reference to the Yukkuri stuff on the left, so if that's not it I have no idea what we're supposed to be seeing.

As far as I know the artist's comment on Pixiv concerning what is "at the feet of the girl in front" is alluding to a splicer corpse that is out of frame. So the only thing to spot in the picture is the yukkuri sign.

They just had to put Hidden Touhou pics and messages. Nice way to kill good BioShock fanart. Especially one that's in anime-style.
