For a laid back guy Sans sure has one hell of a frightening grin. Almost like a skeletons.
It is one of those cough covers(filters/whatever they are called) you can see a lot of people wear when sick in Japan (or during especially dusty construction work). It simply has the smile drawn on it. Just look at the straps at his ears
