I think it's a bit like all those pictures of girls holding condoms in their mouth while having clearly had unprotected sex with semen flowing out of their vagina: It's just enough of a trope that it serves as a visual cue for "masturbating", even when not really used in the act. Or for that matter, "losing virginity" being depicted with gushing blood from the vagina. (Although hypothetically, it's possible they could be, even if not nearly as universally as depicted.)

It was more like he made a passing comment implying that women used tissues to masturbate and she corrected him but yes, my family is weird.
