In real life, yes, but not in (most of) Fate's myriad timelines. Half of the major figures in history were actually girls all along, but maybe had someone like Merlin use magic to convince everyone that she was a boy (reverse trap style).

Not in the Nasuverse, they always where female there... (well outside parallel worlds...) But Nero unlike King Arthur didn't had to hide her gender.
