All it really says is that imagery won't effect her, but imagery and say stabbing her in the heart with a genuinely holy artifact are different things. Immunity to one hardly implies immunity to the other. Just like I can watch imagery of someone being shot without harm, but having a 9mm slug punched through my stomach most certainly would be unpleasant.

We know in Touhou that the power of gods are rather real and can be called upon. Given that it seems rather stupid to assume that only applies to eastern deities and supernatural forces. I see little reason why if an invocation of an eastern god can have very real effects invoking an angel or saint (which was an entire school of medieval magic) could not have equally real ones, including ruining Remi shit.

You do have a point, it depends on how much does it pierce into you... maybe stabbed by a cross would damage her.
But well, Remi's spellcard she gets surrounded by a giant cross with her as the center, i think its mor than just touching it.
Same way she doesnt need to cover herself from the sun really, but it does annoy her.

Moot point, once Sakuya finishes with Sanae there won't be much divine left about her...

Sakuya has the advantage of being awesome and well-liked, and was a PC in both PCB and IN, its just that canon-wise she basically said, "Reimu should be the one taking care of incidents."
