Fair enough. I think from the kanji that the person she's stabbing is actually Kyon, but without knowing Japanese I can't confirm it, nor can I confirm why she's doing it.
The artist didn't seem to tag any character other than Mikuru. So I suppose it's left up to interpretation. Personally, I feel she is cutting open an overly ripe gigantic cherry.

1) Naked Mikuru is always hot. 2) Yandere is fetish fuel for some on its own, but even if not... 3) Many people enjoy the idea of Mikuru getting some violent revenge on Haruhi for how she treats her, though murder is a bit far. 4) When in doubt, see point #1.
Actually, I'd be equally happy if she was stabbing future Mikuru, though it would guarantee a bad ending for her years later. (Or not? Time travel in Haruhi is really wierd.)
