I vaguely remember reading something interesting about that, might've been here on danbooru --
Basically, since Japanese women (and others) have such a strong cultural inclination towards self-control and being professional, the "ah no stop" etc are all about it feeling so good they can't control themselves.
So it's more of a "stop, i can't think straight" rather than "it hurts".
