They most probably punctured it before they inserted the straws, the "how far" being "how deep down the artery"...
Only if it was anatomically correct, of course, because contrary to the popular belief, you can't just "stick in" a straw and start sucking blood, just like you can't stick it into an apple and start sucking apple juice.
The straws' diameter is too large to fit in most veins without rupturing them or stopping the flow, so they probably insterted them in some of the major arteries I listed above... that would also explain why they're plugging the straws with fingers, because the blood pressure in those arteries is high enough for Youmu to bleed to death in minutes.
Again - IF it is anatomically correct, which can be disputed. It's a godddamn drawing, after all.
Does this really count as guro? No arms off, no intestines hanging out, not even a massive amount of blood. Just vampires sticking straws into someone's arteries. It's cruel, but it's really not that graphic.
I though it was sakuya at first, and I was about to be disappointed in the Scarlet sisters. Then I saw it was Youmu and decided that's ok, she's half anyway.
