Personally, I don't see any real intention to cut the breast. It looks like it's just part of the pose. The sleeping girl is set up to look like a corpse with the confetti representing blood and the paper chain resembling her intestines. I'm guessing that's why you tagged it as yandere, but I'm not fully convinced that tag applies here.
In other abuse images, the violence and distress are easily apparent. This is not the case here.
