And so Kyousuke & gang introduced Manami to a world she never thought existed filled with media for nearly every taste.

The way I see it there are two possibilities:

1) Gaming. Kyousuke is playing, but he's struggling. Saori's impressed that he's playing as well as he is, but Kirino is (as usual) less than impressed. Kuroneko, the only real gamer in the group is practically face-palming at how bad Kyousuke's playing. (Note: as a competitive gamer, I have made that face on many occasions). Manami's just surprised at how excited everyone is getting over a game.

2) Porn. Not just any porn, but some weird fetish porn. Kirino, being one of the weirder ones in the room, is more confused at why he's interested in whatever is on the screen than shocked. Kuroneko has always known that Kyousuke's a little weird, but she's pretty dumbstruck at just how weird he is. Saori, being the epic troll that she is, is happy that she now has some good dirt on Kyousuke. Manami is in a state of complete and total shock.

Even though it's not among the most well drawn Oreimo pictures, it is one of my favorites due to how well it parallels Kirino's and Kyousuke's relationship. It could either be a totally innocent picture (Kirino is happy to finally be on good terms with Kyousuke and doesn't want anything to happen to ruin their relationship) or it could be something totally disgusting (Need I even clarify?).

Obviously it's a orgy rhythm game. He's trying to pleasure three 2D girls at once.
