Looks like it. Old Reimu, unaging Alice...and possibly a dead Marisa.

I think this is their childhood.

According to artist comments, the old lady back there isn't Reimu or any Touhou character, and this is the future. Other than that, I can't make out what I'm reading, as to who those two girls are and such.

If I'm reading the source comments right this is just alice hanging around some random old lady and two kids that are cosplaying as Reimu/Marisa and playing pretend.
