Tang went out in late 1944, not really early war anymore. Most of the glaring defects of torpedoes had been worked out by then. More of terribly bad luck than a big flaw in the torpedo itself I'd say, though you could say the fact that it's even possible for the torpedo to do a circular run and hit what fired it is bad design.
