One of the reasons why I tagged this with "1boy" "otoko no ko" is because the original author tagged it that way, and this art can be found under those tags on pixiv.
This art is an April Fool's joke, as this author likes to draw femboys with plump breasts.
However, fanbox does have variations of genderswap and futanari.
I also tagged it with "breast padding" because the author continued to refer to this variation of the art as a boy on Twitter, as did his main audience.
There is also art implying that this version is a dream.
