It's where her accessories are worn. Leg, hand and neck for Chernobyl foot, control rod and Yatagarasu eye. These certainly do look like radiation burns, but I don't really think they are supposed to be some kind of a "toll".

Well, radiation burns aren't exactly the best laughingstock ever, but with the vastly superior youkai metabolism and regeneration it's probably not worse than your skin getting sore after wearing something unpleasant. Utsuho seems quite lively to me in canon.

The first thing that came to my mind was that Okuu got her Yatagarasu powers removed somehow and it didn´t turn out too well for her health and Orin isn´t exactly happy about this.

What matters is that this is very well done. I find Utsuho really beautiful in this one.

Considering that there are no markings on Okuu's fission-foot (which would be just as likely, if not moreso, to have radiation burns), I posit that those are scars from forcible removal of her elephant-foot, control rod, and third eye.

She doesn't need to forcibly remove them. She is seen without them on in two different instances of canon. So she should be able to take them off normally.
