The pixiv tags don't say so. This is Rin Toosaka. The eyes or the ribbons could be just a drawing error. Removing tags.
The ribbons are in fact Madoka's and according to the description, I think it's the relic used to summon Madoka. There also isn't any mention of Homura anywhere so, I'm pretty sure it's just Rin.
