Been a while since I last read the manga and I haven't bothered watching the anime but if I recall it right, Remilia is the First (Yukiteru), Flandre is the Second (Yuno), Meiling appears to be the Third which I think wasn't even supposed to have been there, Patchouli is the Fifth based off the puppets she has as the kid was the only one I remember having them.

So that leaves Sakuya as the Fourth, the cop guy if I remember it right.
