There's several kinds of 'maid' distinction, this particular one was probably based on governess, which essentially serves as live-in etiquette tutor for higher ups' kids.

File this under reasons I prefer chopsticks over western style cutlery, most of the time anyways.

Basic answer for this one - always use the cutlery furthest away from the plate (which is then removed at the end of the course).
