I can't find the conversation currently, but after talking about it with some other users on the Discord we figured that Q was more appropriate. Like you said, it's definitely at least Q, but nothing straight up stood out as E, because of factors like the lack of explicit focus and the wound being completely obscured. This was taking into account both howto:rate and guro rating:q as well.
Visible organs out of their own body like that fall under the "extremely graphic violence" part of howto:rate, that pic should be rated E. Think about it this way: it's as explicit as guro can get. There's no hint of comedy, no surreal ambiguity, he's straight up holding his own guts.
