Uh...I have no idea if this should be S or Q because besides the one guro image, it is a pretty innocent collection of drawings of her (doesn't help it took me a sec to realize Hoojiro is holding her own intestines as opposed to being weirdly blood splattered there). I can see this being S instead of Q as I rated it, but I don't want to rate anything that undeniably needs the guro tag too low.

A person holding their own bloody intestines in clear view should be rated E regardless of how innocent the rest of the image looks. Generally, exposed organs would merit a Q at minimum (such as in body horror).
Edit: @岩戸鈴芽, can you explain why you changed it back to Q? Is it because it is slightly ambiguous whether Hoojiro was actually disemboweled (for example, she may be holding someone else's organs)?

I can't find the conversation currently, but after talking about it with some other users on the Discord we figured that Q was more appropriate. Like you said, it's definitely at least Q, but nothing straight up stood out as E, because of factors like the lack of explicit focus and the wound being completely obscured. This was taking into account both howto:rate and guro rating:q as well.
Visible organs out of their own body like that fall under the "extremely graphic violence" part of howto:rate, that pic should be rated E. Think about it this way: it's as explicit as guro can get. There's no hint of comedy, no surreal ambiguity, he's straight up holding his own guts.

Guidelines for sex-related dialogue can be found in forum #213388, though it would be better for them to be listed in howto:rate. Basically, dialogue that can be understood by English speakers, including translated text, can affect the rating.
Regardless of the dialogue, the collar around the daughter's neck would make this at least S (BDSM is normally Q, but this is on the milder side). I'm not sure whether this combined with the dialogue is enough to elevate it to Q, but I'll keep it as is just in case and let someone else figure it out.

While my gut feeling is that G is more appropriate due to this only really being obvious upon closer observation, I'm not sure if the use of imminent suicide is enough to raise the rating by its very necessity (especially since the situation isn't at all comedic).

If you hadn't said that's what it looks like to you I wouldn't have seen it as that, so I'm also unsure about the application of imminent suicide. As for that tag, in most cases it should probably make something >S, barring comedic circumstances etc. like you mentioned.
