Alright, what the hell is the proper rating here? Pic is tagged R-18G (guro) on Pixiv, but there's nothing really extreme happening to Homura herself. Just the canonical implications of such an action, which I'm not sure are enough to knock it up to Q or E.

Either the suicidal implications or the blood (probably) would be enough on its own for S, but I agree it probably doesn't need to go higher than that given the unrealistic portrayal.

I wouldn't really say it's implied - although you would have to look a bit closer to find out, there's somewhat visible pussy (with mosaic censoring, just underneath the apron) and cream as an innueno for (but not being) cum. It is there but I'm not sure to what extent this would affect the rating besides being Q at minimum.

My thought is: regardless of what function the cream serves, I probably wouldn't be expecting to see that if I just put rating:q in the search bar.

That means it's E already, but the fact that you can see the cream leaking out means that the implication is either after vaginal or after she... inserted some cream in there, there's no simple "whoops messy with cream" explanation here. Both are E, the latter just makes you a bit more.
