Given the sheer salt Sakurai radiated about the CERO incident, I suspect he'd rather cut the cameo completely than give in to the censors.
Another possibility is that CERO kept rejecting it and in the end Mai just couldn't be finalized in time for Terry's release. Like he said in the interview, Smash 4 almost didn't make it to its release date because of all the revisions they had to do for Palutena and Wonder Pink.
