From the looks of it, it was a boy, who the guys thought would look the prettiest as a girl. This is a all-boys school, and the boys really want a girl around to look at, and thus they forced the femboy to crossdress as a girl. How they did the breast i don't know, probably stuffed with some socks and a bra.

... Is there any particular meaning behind "Komatsu" (小松), or is just for the 姫小松 (hime-komatsu) containing 姫?

Looks like the two brothers are back crossdressing other people.
