Yaoi is yaoi. The wiki was edited only one year ago with no discussion as I can see and it´s not even about the cover of the doujin, but the doujin itself. The yaoi is even a central part about the image itself. So adding the yaoi tag is not wrong in this case.

Would somebody who blocks yaoi want this image to be blocked? Quite unlikely.
I can agree to add something like "yaoi_reference" though; that would be quite useful and wouldn't add unnecessary confusion.

I did have yaoi blocked but wanted to search for this picture thankfully i did remember to unblock it. The yaoi is not a central part of the image for sure.
